After downloading this form, please fill in the form completely and fax it back to 1300 881 639 or email to service@schweigen.com.au or mail to PO Box 84, Holmesglen VIC 3148.

Note: Original/Copy of retailers receipt must be returned with the completed service request form. Postage to the manufacturer and all associated costs are the responsibility of the warranty holder.
